Making the diagnosis of ADHD requires symptoms in more than one environment. If you have concerns about ADHD in your child, please call to discuss with your child’s doctor. Your doctor will likely give you these forms to have family and teachers fill out. Feel free to give these forms to teachers before the appointment, and return with forms filled out for review at the appointment. Your doctor will use the information on these forms to determine whether or not your child may have ADHD.
